Hello, I also made the same mistake, during my ui is still pending, the dol send me a msg ask me to complete the pua form, I made a mistake answering a question choose no instead of yes, then hit the submitted button, and it says I’m ineligible, I sent a hearing request and sent a msg explained the situation, but no response, is that meaning I lost my pua? Still pending pua Wtf- my claim was good for 8 weeks now in pending since last saturday when all this started. I entered my last day of work as September 7th 2020 on the initial claim, but I don't know if I still have to request a backdate for the full sum to be released to me once the card comes. It was fine all throughout 2020 but you always have that lingering feeling that wont shake off that one day you go to check and you see a "balance due" notification despite knowing you're fine and have all that you need documentation wise.
